2 comprehensive exams
2 routine cleanings (absence of infection)
1 limited emergency exam & x-ray
2 oral cancer screenings
2 fluoride treatments
4 bitewing x-rays
1 complete series of x-rays / or Panoramic x-ray (every 2 years if needed)
Individual x-rays, as needed
TMJ evaluation
50% off additional cleanings
35% off fillings, core build-ups, night guards, oral surgery, implants, root canals, crowns, bridges, partials and dentures
10% off cosmetic treatment

Program Exclusion and Limitations:

This program is a discount plan, NOT a dental insurance plan. It cannot be used:

In conjunction with another dental plan, dental insurance plan, or financing program such as CareCredit unless discussed with Dr. Storch beforehand
For treatment which, in the sole opinion of Dr. Storch, lies outside her capability
For hospitalization or hospital charges of any kind
For costs of dental care which are covered under automobile or medical insurance
For services of injuries covered under worker's compensation
No refunds
